To the former patients of Dr. Lindy Buck and Heny Taylor III/To The Point, LLC and Bay Pointe Family Practice

As you may know, Lindy Buck,M.D.and Henry J Taylor III, M.D. have decided to close her medical practice. Ohio State retention regulations require that the original medical records accumulated during the term of her practice be maintained for a period of no less than ten years. The doctors have entered into a legal arrangement whereby Iron Mountain, Inc. will not only store the medical records but will also provide release of information services. We understand that many former patients will need to obtain copies of their records and we have established a procedure whereby we will be able to comply with these requests.

The medical records have been moved to our Toledo, Ohio facility. In order for us to provide copies of medical records to patients or the recipient designated by them, we require an authorization signed by the patient or their legal representative. A copy of the authorization is enclosed for your use. Please make certain that the form is completed in its entirety. Failure to do so may result in a processing delay.

There is a $32.50 fee for obtaining copies of medical records. The fee has been established in accordance with both state and federal guidelines. In most cases that fee will cover the cost of copies of all of the pertinent information (all physician notes, lab reports, x-ray reports, EKGs and other diagnostic tests) contained in your record for the most recent two years of treatment. In the cases where the record is very lengthy or you feel the need to obtain a copy of the entire record, you may be billed an excess copy fee. Payment must be submitted before we can process your request.
